Agreement. IO-CORE Website (the “Website”) is an online informational service provided by IO-CORE (“IO-CORE”). The service is offered for your use subject to your acceptance of, and compliance with, the terms and conditions of this disclaimer agreement (this “Agreement”). By accessing the Website you agree to be bound by the terms and conditions of this Agreement. If you do not agree to these terms and conditions, then you may not access or use the Website.

Telephone Numbers and Calls By providing your telephone number to IO-CORE on this contact form, you acknowledge and agree that you may receive a telephone call from IO-CORE, at the number you provided on the online form, to receive information regarding the treatments offered at IO-CORE. Such telephone calls may be placed by an automatic telephone dialing system and/or may contain prerecorded messages. By providing your mobile number to IO-CORE, you further acknowledge and agree that you may receive text messages (SMS) from IO-CORE at the number you provided to IO-CORE. Protected Health Information will not be requested by IO-CORE, if you choose to respond with Protected Health Information you agree to release IO-CORE from any liability related to such disclosure. Be advised that any telephone calls and/or text messages are strictly informational and/or transactional in nature. IO-CORE therefore does not accept liability for any errors or omissions in the contents of email messages, or any responding emails that contain confidential patient information, which arise as a result of email transmission. User of Personal Information. Please refer to the IO-CORE Privacy Policy. Should you provide personal information to IO-CORE, IO-CORE will not release your name, street address, telephone number or email address to any non-affiliated entity without your consent for any non-IO-CORE related purpose.
Informational Purposes Only. The Content, including information provided about health care, is for informational purposes only. No promises or warranties of any kind, either express or implied, are made regarding the quality, accuracy or validity of the Content except where specifically stated.
